SS-31 (elamipretide) is a mitochondrially-targeted tetrapeptide that selectively binds cardiolipin — the unique phospholipid found only in the inner mitochondrial membrane. By stabilising cardiolipin, it restores the structural integrity of the electron transport chain, reduces ROS leakage and improves ATP output without disrupting normal redox signalling. Human clinical work has explored SS-31 in heart failure, age-related muscle weakness, ischemia-reperfusion injury and mitochondrial myopathies. It is one of the few peptides in the energy category with phase-2/3 trial data behind it. Researched outcomes include improved exercise capacity, recovery from fatigue and cellular resilience under metabolic stress. Often stacked with MOTS-c and NAD+ as part of the Mito Stack. Typically run in 8–12 week cycles.

In a randomized dose-escalation trial in adults with primary mitochondrial myopathy, elamipretide (SS-31) significantly improved 6-minute walk distance — the first human evidence that a cardiolipin-stabilising peptide can rescue mitochondrial-driven exercise intolerance.
