Semaglutide is the most widely-prescribed GLP-1 receptor agonist (Ozempic, Wegovy, Rybelsus) and the compound that defined the modern obesity-drug category. It activates GLP-1 receptors in the pancreas, gut and brain — slowing gastric emptying, increasing satiety, and lowering blood glucose and HbA1c. In the STEP-1 trial, weekly Semaglutide produced 14.9% mean weight loss at 68 weeks at the 2.4mg dose, with significant cardiovascular risk reduction in the SELECT trial. The 2025 STEP UP trial pushed the ceiling higher: 18.7% mean weight loss at 7.2mg weekly, a studied dose for people who tolerate 2.4mg well. GI side effects (nausea, constipation) are dose-related and manageable via slow titration over 4–8 weeks. The most evidence-backed, longest-tracked option in the category. Subcutaneous weekly dosing. Available in a 10mg vial size.
Dosage
Start 0.5mg once weekly; double to 1mg after 4 weeks, then step up every 4 weeks (max 7.2mg weekly)

Safety Information
- •Thyroid tumors: Semaglutide caused thyroid C-cell tumours in rodents. Whether it does so in humans is unknown. The branded medicine must not be used by people with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia type 2 (MEN2).
- •Pancreatitis: Pancreatitis has been reported in clinical trials. Stop and seek medical attention if severe, persistent abdominal pain occurs.
- •Gallbladder disease: Gallbladder problems, including gallstones, have been reported, especially with rapid weight loss.
- •Kidney problems: Vomiting and diarrhoea can cause dehydration, which can lead to acute kidney injury. Drink enough fluids.

Clinical Research
STEP 1 enrolled 1,961 adults with overweight or obesity without diabetes. At 68 weeks, once-weekly semaglutide 2.4mg produced 14.9% mean weight loss versus 2.4% with placebo; both groups received lifestyle intervention.

SUSTAIN-6 enrolled 3,297 patients with type 2 diabetes and high cardiovascular risk. Over 104 weeks, cardiovascular death, nonfatal myocardial infarction or nonfatal stroke occurred in 6.6% receiving semaglutide 0.5mg or 1mg weekly versus 8.9% receiving placebo (hazard ratio 0.74; 95% CI 0.58–0.95); the trial met its prespecified noninferiority criterion. These results concern this studied population, not proof of benefit independent of weight or glucose changes.
